On Your Feet Soldier/Survivalist Clarification Please?

I am trying to run through and get the onyx medal for "Survivalist," which says to complete a chapter without dying. Every time I have done that, the ribbon "On Your Feet Soldier" will pop up. So my guess is that they are connected. Another thing, if I go DBNO in the chapter, the ribbon won't pop up at the end, and I don't get credit towards "Survivalist" either. However, I have noticed during co-op play that when all characters have died and we have to start a section over in the chapter, I still get the ribbon if I don't get DBNO. Is "On Your Feet Soldier" glitched or can somebody explain to me how the medal/ribbon truly works?

I think the medal 'Survivalist' is for completing a chapter without dying or going DBNO. I don't understand how you can of all died but not gone DBNO? You will receive credit if you don't go down or die no matter what happens to the other 1-3 players (so if you're player A and player B dies it won't affect you).

Okay, it seemed to have done it again, I was on campaign mode on insane and we were on the chapter where we rescued Dizzy. We all ended up dying, having to do the chapter over again, and when finished, it unlocked for me.

You all died and started the chapter over again resetting you death. So my bet is that you replayed the chapter the second time and didn't die or go DBNO.

You don't go down on insane, you just die.
