Probably something wrong with your last saved game, check the chapters and see if all chapters are completed!!!

Yea i checked this when i noticed and throughout the whole game ive got the symbol for completing it on insane except the last 3 chapters on act 5, im currently completing it on casual to see if it lets me select insane from my saved game afterwards, if not i wont be a happy bunny some bits arent exactly easy on insane!

Did you happen to notice if your collectibles as well as any progress you had for achievements were set back to zero? When I had a campaign reset I lost the ability to choose insane. I had to play through the campaign on casual to unlock it again.
    Campaign>Horde>Arcade & MP>Beast
