Horde: after player death, player does not join next wave

Hi,

during the last two days we played ~ 120 waves of Horde. It happened 'once' that one of my friends, we were playing at his place, three xboxes, did not join the next wave after dying the wave before. We did not spot anything unusual as the game just went on as expected, besides 'him' not joining. Within that round we actually all died and also quit the session, so I cannot tell whether he would have stayed 'permanently dead' or not.

Were you in a Party?: GOW2 ingame party, not XBL party.
If so, how many were in your party?: Me and two friends locally, playing a private match with two other players via internet (probably not host)

How did you leave?: N/A
Ranked or Public or Both?: Public
Was this in your first match or after transition?: somewhere around beginning of wave 13
What was happening right before the issue occurred?: Grubs got killed, player died, wave ended
What is your NAT type?: Open

As said, happened once only.

I have experienced this, die during wave, team succeeded in finishing wave, at start of next wave I am left dead on the ghost camera.
I do have the option of buying back in.
OR
If I remain dead for the next wave the wave after that I will respawn in.
This is nuts.
I end up quitting the game and joining a new one.
