Hi,
during the last two days we played ~ 120 waves of Horde. It happened 'once' that one of my friends, we were playing at his place, three xboxes, did not join the next wave after dying the wave before. We did not spot anything unusual as the game just went on as expected, besides 'him' not joining. Within that round we actually all died and also quit the session, so I cannot tell whether he would have stayed 'permanently dead' or not.

What was happening right before the issue occurred?: Grubs got killed, player died, wave ended
