I have completed every chapter and act of the game on insane, with four players, using arcade mode. I have been awarded the Onyx Tour Of Duty medal but my Warmonger medal remains at silver.
A lot of people seem to have this problem, especially since the announcement of the impossible mission. This medal needs fixing.
EDIT: Never mind I just had to play a previous level, doh, even though i was sure i'd done em all.
