This idea may need refining a little as I've literally just thought of it.
Basically, my idea is this:
When you shoot and hit an enemy, you enter into a 1v1 battle with them. This means that every other player is made invisible from your and your enaged enemy's perspective. Every other player in the server will also not be able to see you or your enaged enemy. Collisions and damage will be removed too, so that you cannot accidently walk into an enemy you cannot see and you cannot accidently kill an invisible enemy with spam tactics etc. Obviously, visual details of weapons and weapon's fire will also be made invisible.
This battle will last until a timer ends or a frag is made (or a number of frags, depending on a server side value set in the mutator's config), at which point both of you die and respawn, with all other players visible to you again (unless they are fighting their own 1v1 battle, of course). Both people dying would be nessasary to avoid the winner re-appearing inside of somebody not currently fighting a battle, but of course this death will not lose the victor any points. Maybe the winners should also spawn with the weapons, armour, health and damage amp they die with.
Also, no shields, damage amp or healths should be available until a 1v1 battle has started. These powerups should then appear for those respective people only (this probobly needs re-thinking a little - timing the powerups is a skill which would be negated slightly with this system, especially if the battle timer or the number of frags required to win a battle is set to a low value. I'm sure a better powerups system could be implemented if enough thought is put into it).
The only problem I see with this is having to fight the same person over and over again. The likelyhood of finishing a battle at roughly the same time as somebody else would be quite low, so you may end up meeting the same person all the time after spawning. Perhaps not allowing the two people to damage each other again until one of them finishes their next battle, or a time limit expires (with a visual change, like a glowing skin, to make it clear not to engage each other agaub). This would give you a chance to find and fight new people.
The whole point of this is to combine the good things about 1v1 with the good things about fragfests. You can fight many people but at the same time you don't have to worry about anybody else butting in whilst you are trying to kill somebody.
