How about making a little mutator, that displays your image instead of the model image, when talking.
It should work something like that:
You select a picture (right size), and then the path is saved in your user.ini
When you join a server with the mutator, then the server reads the entry in your user.ini and uploads the little image. Then it displays your image while talking.
It would be very cool, if someone makes this. You could also implement this in ut2k3 ;)
