I've downloaded the 2.2 ttm file: ttm2003v22.ut2mod. Please tell me how should I install it?

Can you use it in an offline match? Can you select this as a mutator? I ask this, because my mouse X-hair gets f***** up when I enter a TTM-server.

Yeah, you can use it off line if you have it installed on your system. Also, make sure to add the serverpackages entry in your ut2003.ini.

To adjust the cross hairs, press F7, and look under the player settings menu.
